The Preventative Integrated Care Service (PICS) is an innovative community-based service providing rapid access to intensive specialised medical management, delivered by a team of medical, nursing, and allied health clinicians. We focus on supporting people with chronic conditions who require multi-disciplinary input to avoid a potential hospital presentation or admission.

About you
Our ideal candidate will be someone who can demonstrate the following:
- Demonstrated high level knowledge of contemporary preventative and hospital avoidance service delivery with the ability to integrate evidence-based knowledge into practice.
- Demonstrated ability to lead a multidisciplinary team utilising the principle of contemporary human, financial and material resource management.
- Demonstrated high level oral and written communication and interpersonal skills to facilitate equitable and effective engagement with staff and internal and external stakeholders.
- Demonstrated ability to provide strategic leadership for service planning and service improvement for the service.
- Demonstrated ability to provide clinical leadership for the development of professional standards for Prevention and Hospital Avoidance practice in West Moreton Health.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
- Appointment to this position requires proof of qualification and registration or membership with the appropriate registration authority or association within Australia. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
